Registered NDIS provider – services for adults

Group / Centre Based Activities

Our fun filled and fulfilling day centre is focus on empowering and enhancing the quality of life of each participant we support.  We are committed to choice of programs in a holistic person centered approach that includes the following items:

  • Sport and Recreation
  • Life Skills: housekeeping, cleaning and gardening
  • Budgeting, Shopping and Handling Money
  • Sensory Activities/Fine Motor Skills
  • Computer skills and navigating the internet
  • Art Programs
  • Personal Care and Grooming

Support Connection

(Level 1)

This support is to help the participant understand the NDIS plan and build your ability to connect with informal, community and funded supports enabling you to get the most out of your plan and pursue your goals.

Coordination of Supports

(Level 2)

This support will assist you to build the skills you need to understand, implement and use your plan. A support coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community. Your Support Coordinator will assist you to negotiate with providers about what they will offer you and how much it will cost out of your plan. Support coordinators will ensure service agreements and service bookings are completed. They will help build your ability to exercise choice and control, to coordinate supports and access your local community. They can also assist you in planning ahead to prepare for your plan review. Support coordinators will assist you to optimize your plan ensuring that you are getting the most out of your funded supports.

Specialist Support Coordination

(Level 3)

This is a higher level of support coordination usually for more complex situations.

Specialist support coordinators will support you to manage challenges in your support environment which may include health, education, or justice services. Specialist support coordination aims to reduce barriers to implementing or using your NDIS plan. A Specialist Support Coordinator will be funded where there are additional high or complex needs in your situation and will be a qualified and experienced practitioner such as an Occupational Therapist, Psychologist or Social Worker.

Assist – Life Stage, Transition

(Psycho-social recovery, mentoring, peer support)

This support includes psychosoical recovery/coaching, mentoring, peer support and skill development that aims at focusing on individual needs. The support aims to provide assistance with accommodation, decision making, budgeting, planning short/long term goals and supporting you through your life journey. This support enables you to stay in the comfort of your home and participant in community activities at your leisure and of your choice.

We strive to support our participants and their families to live and thrive in an inclusive and enriched community.

Flexible services are available via Skype, Zoom, WebEx, MS Teams & Zoom calls, home visits and weekends.

Where do I get a Support Coordinator from?

You can look up registered support coordination providers using the Provider Finder tool in my place. The Provider Finder will let you search for support coordination providers that are located near you. Every three months the NDIA publishes a list of registered providers in your state on the NDIS website.
